It is a high-grade recommendation with unanimous consensus among the experts who wrote it.\r\n      <\/p>\r\n      <p>\r\n        One honest caveat is worth adding, because almost nobody states it: <strong>the recommendation is strong, but the available evidence is weak<\/strong>. The guideline itself acknowledges that there are no randomised comparative studies on compression and pain in lipedema, and the only published trial is a pilot study with six patients. That an indication is well established does not mean it has been precisely measured.\r\n      <\/p>\r\n    <\/div>\r\n\r\n    <div class=\"pclac-duo\">\r\n      <div class=\"pclac-duo-card si\">\r\n        <h3>What it does<\/h3>\r\n        <ul>\r\n          <li><strong>Reduce pain<\/strong> in the affected areas. It is the best-supported indication and the aim stated in the guideline.<\/li>\r\n          <li><strong>It may improve<\/strong> the feeling of heaviness and other subjective symptoms.<\/li>\r\n          <li><strong>It may reduce<\/strong> the tendency to bruise, according to that same six-patient pilot study.<\/li>\r\n          <li><strong>It could improve<\/strong> quality of life when combined with exercise. The guideline puts this in the conditional, and so do we.<\/li>\r\n        <\/ul>\r\n      <\/div>\r\n      <div class=\"pclac-duo-card no\">\r\n        <h3>What it does not<\/h3>\r\n        <ul>\r\n          <li><strong>It is not a treatment for reducing volume.<\/strong> Lipedema is not, primarily, an oedema disease, and compression does not remove the affected tissue.<\/li>\r\n          <li><strong>There is no evidence that it halts progression<\/strong> of the condition. Neither the guideline nor the literature supports that.<\/li>\r\n          <li><strong>It does not replace surgery<\/strong> when surgery is indicated, nor does it remove the need to consider it.<\/li>\r\n          <li><strong>It does not cure lipedema.<\/strong> The condition is chronic and has no cure.<\/li>\r\n        <\/ul>\r\n      <\/div>\r\n    <\/div>\r\n    <p class=\"pclac-tnote\">If you came here looking for how many centimetres compression takes off, the honest answer is that this is not what it is for. And the correct answer is that <strong>nobody can tell you that from a website<\/strong>, ourselves included.\r\n      <\/p>\r\n      <p>\r\n        It is not evasion, it is what the guideline states expressly: <strong>a fixed compression class should not be assigned simply because someone has lipedema<\/strong>. It is individualised according to the location, the findings on examination and the intensity of your symptoms. And it adds a second, striking criterion: always prefer <strong>the lowest class that achieves sufficient relief<\/strong>.\r\n      <\/p>\r\n      <p>\r\n        The reason is practical. A higher class does not equate to a better result, and it does equate to a garment that is harder to put on and to tolerate. The garment that works is the one you actually wear every day.\r\n      <\/p>\r\n      <div class=\"pclac-callout\">\r\n        <p><strong>If you see tables assigning a class by lipedema stage:<\/strong> they exist, and they come mainly from the 2021 United States consensus. The 2024 European guideline, which is more recent, expressly forbids it. We are not going to give you a number, and the reason is that <strong>there is no hourly regimen published in the clinical guidelines<\/strong>. Not in the 2024 European one, not in the United States consensus. The figures in circulation have no primary source behind them.\r\n      <\/p>\r\n      <p>\r\n        What is established is the principle underneath: <strong>the regimen is set by your doctor or your therapist<\/strong> according to your symptoms and your tolerance, and the criterion guiding the decision is adherence. Put another way, a garment you actually wear daily relieves more than the perfect garment you end up leaving in a drawer.\r\n      <\/p>\r\n      <p>\r\n        That has a practical consequence: if your garment is unbearable, the answer is not to put up with it. It is a reason to review the class, the fabric or the type of garment in consultation.\r\n      <\/p>\r\n    <\/div>\r\n  <\/div>\r\n<\/section>\r\n\r\n<!-- ============================= SUMMER ============================= -->\r\n<section class=\"pclac-section\">\r\n  <div class=\"pclac-container\">\r\n    <div class=\"pclac-prose\">\r\n      <div class=\"pclac-eyebrow\">The question every June<\/div>\r\n      <h2>Compression <span>in hot weather<\/span><\/h2>\r\n      <p>\r\n        In Spain this is not a minor detail: it is four or five months a year in which wearing compression goes from uncomfortable to difficult. And it is when most patients abandon it, precisely when the heat is making heaviness and swelling worse.\r\n      <\/p>\r\n      <p>\r\n        There is no magic solution, but several things help and are worth raising in consultation before the heat arrives:\r\n      <\/p>\r\n      <ul>\r\n        <li><strong>Review the class before summer.<\/strong> If the class you wear is the highest you can tolerate in winter, in August you probably will not tolerate it. Dropping to a class you can actually use is better than using none.<\/li>\r\n        <li><strong>Ask about summer fabrics and colours.<\/strong> Manufacturers make thinner, more breathable versions in lighter shades; they do not change the compression, they change how it feels.<\/li>\r\n        <li><strong>Have more than one garment.<\/strong> In hot weather you wash daily, and a single garment makes rotation impossible.<\/li>\r\n        <li><strong>Put it on when you get up<\/strong>, before the heat and standing have swollen the leg. With the leg already swollen it is harder to put on and it presses more.<\/li>\r\n        <li><strong>Water is your ally.<\/strong> Swimming or walking in water provides a natural compression effect through hydrostatic pressure, and those are the hours when you can go without the garment and still get the benefit.<\/li>\r\n      <\/ul>\r\n      <div class=\"pclac-callout\">\r\n        <p><strong>What we do not recommend:<\/strong> abandoning it without telling anyone and picking it up again in October. If summer makes it impossible for you, say so in consultation and it gets adjusted. A regimen you can follow for nine months is better than one you abandon for three.<\/p>\r\n      <\/div>\r\n    <\/div>\r\n  <\/div>\r\n<\/section>\r\n\r\n<!-- ============================= HYGIENE AND REPLACEMENT ============================= -->\r\n<section class=\"pclac-section white\">\r\n  <div class=\"pclac-container\">\r\n    <div class=\"pclac-prose\">\r\n      <div class=\"pclac-eyebrow\">What nobody explains<\/div>\r\n      <h2>Care, hygiene and replacement<\/h2>\r\n      <p>\r\n        A compression garment is a medical device, not a stocking. It loses its properties with use, and a garment that has stretched out does not compress: it only gets in the way.\r\n      <\/p>\r\n    <\/div>\r\n    <div class=\"pclac-check\">\r\n      <ul>\r\n        <li><strong>Wash it daily.<\/strong> Sweat and skin oils degrade the elastic fibres, and washing also restores some of the fabric's recovery.<\/li>\r\n        <li><strong>Keep at least two garments in rotation.<\/strong> One on, one drying. With a single garment the above is impossible.<\/li>\r\n        <li><strong>By hand or on a delicate cycle<\/strong>, with neutral soap, no fabric softener and no bleach, both of which attack the elastic.<\/li>\r\n        <li><strong>Never in the tumble dryer or on a radiator.<\/strong> Direct heat destroys the fibres. Dry flat, out of the sun.<\/li>\r\n        <li><strong>Mind your nails and rings.<\/strong> A snag in flat-knit fabric can ruin the garment.<\/li>\r\n        <li><strong>Replace it when it stops compressing.<\/strong> If the garment slides on easily and no longer holds, its working life is over, even if it looks fine. It is not. A badly indicated or badly fitted garment can do harm, and there are situations in which it should not be used at all.\r\n      <\/p>\r\n      <p>\r\n        Compression <strong>is not suitable in every case<\/strong>. There are medical contraindications, among them advanced peripheral arterial disease and decompensated heart failure, which is why prescribing it always requires a prior medical assessment.\r\n      <\/p>\r\n      <p>\r\n        And in use, the guideline insists on respecting the rules of application and caring for the skin daily. With bandaging, and with poorly fitted garments, <strong>constriction, nerve injury and blistering<\/strong> have been described.
